Community based Transitional Rehabilitation and long-term care services for persons with acquired brain injury.  Transitional Rehabilitation is a comprehensive, transdisciplinary program including PT. OT, SLP, neuropsychology and behavioral individualized treatment in a setting that can provide 24-hour supervision, care and access to medical and neuropsychological and behavioral supports as well as recreational and vocational activities. Does not accept Medicare or Medicaid insurance. For adults.

Long term care available in Assisted living and Supported Living environments.
